How does a team request a playoff sub?

In order to request a sub for the playoffs, teams need to submit an online form to the office.

Depending on your city, it either needs to go to:

https://www.underdogseattle.com/playoff-substitute

https://www.underdogportland.com/playoff-substitute

Once submitted, the office will review their request. Teams have to submit their request ahead of time. They can't show up to the court and request one.

You cannot approved a playoff sub yourself at the courts.
